Carenextion, powered by Senior Community Services, supports families and caregivers of elderly and/or disabled loved ones by managing care coordination.
Eldercare Locator is a public service provided by the Administration on Aging (AoA) that connects older Americans and their caregivers with trustworthy local support resources.
Long-Term Care.Gov includes a range of services and supports individuals to meet their personal care needs.
Minnesota Adult Abuse Reporting Center provides a toll-free number the general public can call to report suspected maltreatment of vulnerable adults, (844) 880-1574.
Minnesota Long-Term Care Partnership is an option in Minnesota where you can purchase a special type of long-term care policy that allows you to protect assets, if at a later date, you need to apply for Medical Assistance.
Financial:
Community Action Partnership (CAP) assists with payment of energy bills, facilitates the repair or replacement of homeowners malfunctioning heating system and helps navigate the process of applying for financial assistance to prevent eviction.
Investor.gov Explore a wealth of information about retirement topics such as employer-sponsored plans, managing lifetime income, avoiding retirement fraud and understanding senior specialists' designations.

AARP - Health Center offers a variety of resources to health, including: healthy living, Medicare resource centers, health insurance, dementia, and food and nutrition.
Alzheimer's Association Minnesota-North Dakota Chapter provides no-charge support, education programs, support groups and training for individuals and families in Minnesota impacted by Alzheimer's disease and other dementias.
Alznavigator is a digital tool that allows families and individuals affected by dementias to create a step-by-step action plan.
Assistance for Seniors - USDA The USDA Food and Nutrition Service offers resources for food and nutrition assistance to seniors.

Foot Care with the Foot Nurse The Foot Nurse provides a foot assessment, trim toenail's, thins over-grown nails, files corns/calluses and manages fungal toenails. The Foot Nurse can assist with in-grown nails and diabetic foot care. Foot care is important for your overall health. Let us help you maintain your healthy feet and promote mobility. Friends & Co Meaningful connections for older adults. They offer coffee talks, cards connect, visiting companions, and phone companions.
Help At Your Door provides grocery shopping assistance by delivering food directly to your home.
Institute on Aging provides online educational resources geared to enhance the quality of life for aging adults. They host a 24-hour toll-free Friendship Line for people 60+ and adults living with disabilities. (800)971-0016
MNChoices Carver County Health & Human Services uses this assessment and support application system to determine eligibility for a wide variety of programs and services for individuals of all ages.
Supplemental Nutrition Assistance Program (SNAP) helps low-income Minnesotans get the food they need for nutritious, well-balanced meals.
The Humanity Alliance offers free or freewill donation weekend meal delivery. Orders must be placed by the end of the day Monday to receive delivery on the following Friday.

Seniors Helping Seniors pairs those who need assistance with light housekeeping, cooking, shopping, companionship, yard work, and small repairs, with fellow seniors.

Carver County Community Development Agency (CDA) provides affordable housing opportunities for residents throughout the County, including over 600 rental units for working families and seniors.

Minnesota Star Program A System of Technology to Achieve Results (STAR) is located within the Minnesota Department of Administration and helps all Minnesotans with disabilities gain access to and acquire the assistive technology they need to live, learn, work, and play.
Transportation:
WeCAB is a network of volunteers who provide safe, affordable, and flexible supplemental transportation for medical appointments, groceries, social events, etc.

Carver County Veterans Services ensure that all Veterans, their families, and survivors receive all of the benefits and services they so well deserve for the hardships they have endured.
Minnesota Veterans Services is a cabinet-level state agency dedicated to serving Veterans and their families by assisting them in securing state and federal benefits, and by providing programs and services relating to higher education, benefits, burial claims, outreach, and Veterans preference.
Veterans Affairs runs programs benefiting veterans and members of their families. It offers education opportunities and rehabilitation services and provides compensation payments for disabilities or death related to military service, home loan guarantees, pensions, burials, and health care that includes the services of nursing homes, clinic, and medical centers.
Vision:
Minnesota State Services for the Blind (SSB) offers counseling, training, and referrals to help seniors find ways to adapt to changes in their vision and improve their quality of life and their ability to live independently.
Minnesota Lions Vision Foundation has been supporting the blind and visually impaired since 1960. Assistance includes the used eyeglasses program and gift of sight program.
Vision Loss Resources is a nonprofit of training, classes, social activities and support groups for people with vision loss living in Minneapolis-Saint Paul area.
